3D rings are the real topic of Garrison’s systems. The “v” shape of a ring that fits in the interproximal area allows a good fit between the cavity margins and the matrix in the buccal and palatal walls. This results in easier positioning of the composite masses close to the cavity margins, and final remodeling (usually necessary at the time of removal of the matrix) will be very minimal.

The 3D rings do a beautiful job of preventing flash by pressing the sectional matrix band against the tooth to be restored. With the prior ring generation, there were some teeth whose anatomical shape resulted in pop-off. With the addition of the new 3DXR ring, that almost never happens. It has become the go-to ring, especially for more challenging situations.

Posterior direct composite restorations continue to be among the treatments that clinicians provide on a frequent and regular basis. Challenging enough given the limited visibility and access that are characteristic of the posterior area, these restorations typically require technique sensitive adhesive protocol, including incremental layering and curing of the composite. Even when meticulous placement techniques are implemented, posterior direct composite restorations can be prone to polymerization shrinkage and microleakage that lead to secondary caries and post-operative sensitivity.
